Situation: You successfully received your EAD card, but the card was later lost or stolen. 

Resolution: You must apply for a replacement EAD card with USCIS. Log into your myUSCIS online account and file Form I-765, Application for Employment Authorization. Be prepared to pay a new I-765 filing fee. 

1. Select the correct EAD category for your employment type
2. Select “Replaceent of lost, stolent or damaged employment authorization document” as the reason for filing
3. Include:
   • Your most recent Form I-20 showing your OPT / other employment recommendation or approval with dates
   • I-797 Approval Notice, which you can find under the “Documents” tab in your USCIS online account
   • Brief description of the reason for the replacement
   • A copy of your previously issued EAD, if you have one

*If your EAD card was lost in the mail by the US Postal Service, try to obtain a copy of a signed letter from your local Post Office confirming that your EAD was lost or not delivered. USCIS may consider waiving the fee with this evidence that the card was never received due to circumstances beyond your control.
